|
To be eligible for Hospice Services, patients,
families and/or guardians must have the desire
to receive palliative care. The attending
physician and the Generations Hospice Team will
provide support and comfort measures to patients
that meet the medical criteria of having an
estimated life expectancy of six months or less.
The Generations Team will conduct an evaluation
initially to determine eligibility of your loved
one.

Generations Hospice participates in the Medicare
and Medicaid programs, accepting both as payment
for services. This includes visits from the
hospice team, medications and equipment and
supplies related to the patients terminal
illness as identified in the Plan of Care. |
Generations
Hospice offers palliative care and support
to all terminally ill patients and their
families regardless of gender, national
origin, race, color, creed, sexual
orientation, disability, age or place of
residence. In keeping with our mission
statement and philosophy, Generations
Hospice is committed to providing the best
care available to you and your family while
giving back to our community.
Special arrangements to communicate with
patients/ caregivers with limited language
proficiency and hearing or speech
impairments will be made at no cost to the
patient upon request.
